Passports

The Clerk's Office is an Agent for NEW passports (Form DS-11).    Passport application forms may be obtained at the Clerk's Office or from the Department of State website: www.travel.state.gov. 

Make a 30-minute passport appointment,  Monday through Friday, 9:00 a.m. to 3:30 p.m. Please make one appointment per application.

Basic requirements:
Form DS-11 (must be completed before your appointment)
Proof of Citizenship 
Identification
Photograph - Photo Service Available at the Clerk's Office
Payment to the U.S. Department of State (one check per application)
Payment to the Clerk's Office (Separate check from the U.S. Dept of State check, or cash)

PASSPORT FEES: CHECK OR MONEY ORDERS ONLY - NO CASH

Passport Renewals: 

The applicant prepares and mails adult passport renewals using the DS-82 form, which the Clerk's Office cannot accept.  Passport renewals for minors or passports that have expired for five (5) or more years must be completed as a new passport using the DS-11 form and executed by a Passport Acceptance Agent.
