The Clerk's Office is an Agent for NEW passports (Form DS-11). Passport application forms may be obtained at the Clerk's Office or from the Department of State website: www.travel.state.gov.
Click here to make a 30-minute passport appointment, Monday through Friday, 9:00 a.m. to 3:30 p.m. Please make one appointment per application. Appointment times will be limited eight (8) weeks before an election.
Both parents must be present. When both parents are unable to appear with the minor applicant, Form DS-3053, Statement of Consent: Issuance of U.S. Passport to a Minor Under Age 16, notarized and signed by the non-applying parent, with front and back photocopy of their ID must be presented, Court Order granting sole legal custody to the applying parent, or death certificate of the non-applying parent. Passport Renewals:
The applicant prepares and mails adult passport renewals using the DS-82 form, which the Clerk's Office cannot accept. Passport renewals for minors or passports that have expired for five (5) or more years must be completed as a new passport using the DS-11 form and executed by a Passport Acceptance Agent.
